Speech Therapy
The ability to communicate is essential to an individual's self worth. Divine Health Care's Therapist will work to restore our clients self confidence by aiding in the abatement of speech disorders.
Home Evaluations of patients with a physician's order for home speech therapy services will include, but not limited to, the following:
- Obtaining a history including communication problems.
- Assessing the emotional and physical problems relating to communication problems and the potential for rehabilitation.
- Assessing the patient, family's caregiver's ability to follow therapy instructions.
- Assessing the home environment and equipment for safety.
- Evaluating appropriateness of skilled care.
- Completing the physician's Plan of Treatment for speech therapy, including the amount, frequency and duration of specific procedures and modalities.
- Setting realistic measurable short and long-term goals with the patient and family.
- Initiating discharge planning.
- Completing documentation on appropriate forms.
- Instructing and assisting the patient, family and caregiver in following the prescribed treatment program.
- Assessing the patient's progress and ability to tolerate the prescribed treatment program.
- Re-evaluating the patient's speech therapy needs and implementing physician ordered changes.
- Checking home environment, adaptive language equipment and devices or oral prostheses for safety and proper working order.
- Completing documentation of visits reflecting skilled care as defined in plan of treatment.
Occupational Therapy
Our Occupational Therapist work with our clients and their families to ensure that recovery from occupational related injuries is quick and complete.
Occupational Therapy Services include:
- Assessing the client's physical status, functional level and potential for rehabilitation.
- Assessing the client's ability to perform activities of daily living independently and safely.
- Assessing the client's, family's, and care giver's emotional status ability to follow therapy instructions.
- Assessing the home environment and equipment for safety.
- Evaluating appropriateness of skilled occupational therapy care.
- Setting realistic measurable short and long-term goals with the client, family, and caregiver.
- Completing the Plan of Treatment for occupational therapy, including the amount, frequency, and duration of specific procedures and modalities.
- Initiating discharge planning.
- Completing documentation on appropriate forms.
- Assessing the client's progress with activities of daily living.
- Re-evaluating the client's occupational therapy needs and implementing physician ordered changes.
- Checking home environment and equipment or appliances for safety and/or proper working order.
- Assessing the client's endurance and ability to tolerate the prescribed therapy.
- Monitoring the client's physical functioning with special regard to the upper extremities or hands.
- Completing documentation of visit reflecting skilled care as defined in Plan of Treatment
Physical Therapy
Physical Therapy involves working with the client, their family, and their physician to ensure that the physicians prescribed treatment program is followed correctly. We feel that healing happens quicker in ones own personal environment, so we will ensure that our clients home environment is safe and in proper working order.
