Medical Social Services
The Medical Social Worker is a qualified professional person who provides Medical Social Services to patients in the home with the physician's orders and under the direction of the Director of Professional Services.

Provides rehabilitative and supportive casework geared to restoring patients to their optimum level of social and health adjustment. This includes assisting patients and their families to understand, accept and follow medical recommendations.

Assist patients in the utilization of community resources which includes: counseling, financial assistance, placement assistance, arrangement of meals, transportation, assist with discharge planning, social worker interventions, and coordination of available community resources.

Assist patients and their families with personal and environmental difficulties which predispose them to illness or interfere with obtaining maximum benefits from medical care.

Assess the social and emotional factors of the patients to estimate the patient's capacity and potential ability to cope with problems of daily living.

Assists the patient, family and other caregivers understand, accept and follow medical recommendations and provide services planned to restore the patient to optimum health.

Consult with physician and other health care team members for the purpose of assisting them to understand significant social, emotional, and environmental factors related to the patient's health problems.
